Iridoschisis Demonstrated by Ultrasound Biomicroscopy in a Patient with Vogt-Koyanagi-Harada Syndrome-Case Report

Iridoschisis is the separation of iris into two layers. It is often associated with glaucoma. We report a case of iridoschisis in a 26-year-old male with Vogt-Koyanagi-Harada Syndrome. Elevated intraocular pressure with iris bombe was noted in the right eye. High frequency ultrasound (ultrasound biomicroscopy) of the right eye demonstrated splitting of the iris stroma resulting in cystic structures. The posterior iris pigmented layer was intact. Extensive posterior synechia and anterior iris bowing with angle narrowing were also seen. Multiple surgical peripheral iridectomies were performed and the intraocular pressure of the right eye was controlled. Follow-up ultrasound biomicroscopy showed a resolution of the cystic structures. Iris pathology showed a separation of the iris stroma without inner epithelial lining, which is consistent with iridoschisis.
