Adverse effect of positive end-expiratory pressure on pulmonary perfusion and arterial oxygenation

A case with unilateral pulmonary infiltration and severe hypoxemia is described in which positive end-expiratory pressure increased the hypoxemia. Concomitant shifts in regional perfusion provided an explanation for this phenomenon; possible mechanisms are discussed.
