Objectives and background: Due to the high frequency, inguinal hernia is probably one of the earliest pathologies ever observed. It is a protrusion of the abdominal cavity contents through the inguinal canal, and can be caused by various factors. The incidence of inguinal hernia is variously estimated. Mainly reports estimate the frequency of encounters in children from 0.8 % to 4.4%. The aim of this study was to analyze the management and outcomes of inguinal hernia repair in children. Patients and Methods: From December 1990 until December 2014 case records of 6826 patients who underwent surgery for inguinal hernia were retrospectively reviewed. Data were collected from archives of the Department of Pediatric Surgery, University Hospital Split. Each patient was analyzed for following parameters: sex, age, localization of hernia, intraoperative or postoperative complications, recidive and surgical method. Results: The study included 6826 patients operated on for inguinal hernia. The data analysis showed a significantly higher incidence of inguinal hernia in boys than girls: 69.30 % in favor of boys. The average age was 3,5 years. Inguinal hernia occur more often on the right (59,5%) than on the left side (33,72%). Bilateral hernia was found in 6,78% of the children. Marcy repair was the most commonly used surgical procedure (6536 patients; 95.76 %), while 272 children (3,98%) were operated using Ferguson procedure. Overall recurrence rate was 0.43%. The recurrence rate was 0,36% for Marcy repair and 1,83% for Ferguson repair. Conclusions: Given the number of patients treated under the above- mentioned period, it is obvious how important inguinal hernia is in surgical practice. We can conclude that Marcy inguinal hernia repair has excellent outcome with low percent of recurrence.
