Projekt programu kompleksowej opieki nad chorymi z niewydolnością serca (KONS)

The project of comprehensive care program for patients with heart failure (HF), arguments for it, its goals, logistics and the description of the pilot phase have been shown in the article. The proposed model of care in patient with heart failure is the next example of the "disease management" program, after the comprehensive care program for patients with myocardial infarction and other health programs of the National Health Fund implemented in 2017. Due to the large number of patients and the multimorbidity accompanying heart failure, it is necessary to conduct a pilot phase prior to full program implementation. The final outcome of the pilot phase should be to determine how changes associated with the new program may affect the frequency of rehospitalization of HF patients, their quality of life and finally their survival.
