Summary Every year, in France, several deaths resulting from the ingestion of toxic mushrooms are notified. Almost all are linked to cyclopeptide poisoning responsible for a toxic hepatitis. Aminata phalloides (death cap), Aminata virosa (destroying angel) and Aminata verna (fool's mushroom) are the best known and the deadliest amatoxin-containing mushrooms. We report the case of cyclopeptide poisoning following the consumption of Lepiota brunneoincarnata confused with Macrolepiota procera (parasol mushroom). The patient recovered well after symptomatic treatment. The prognostic factors of such poisoning and the rationale of various treatment modalities, including the role of liver transplantation and of the molecular adsorbant recirculating system (MARS™), are outlined.
