The gastroesophageal reflux disease (GERD) is a s e rious health problem due to high prevalence and need of mai n tenance therapy. Gastric acid secretion inhibition is the elect i ve therapy and only in patients with GERD withou t esophagitis, or with slight grade of esophagitis, could try th e rapy with dietetic recommendations and «no antisecretors » drugs. Proton pump inhibitors have showed more efficacy tha n H 2-antagonists in esophagitis prevention and healing, i n symptoms improvement and in decrease complications o f GERD. The asks to answer in an early future are : •How much time is sure gastric acid secretion inhibition ? Actually, follow-ups only have 10-12 years . •Could support our health system disease cost? Cost-eff i cacy study from another health system are in favour o f surgery . Therefore, we believe that surgery will turn to be electiv e treatment in many of these patients in our country .
