38 – OSTEOPOROSIS AND METABOLIC BONE DISEASE IN LUPUS

In patients with systemic lupus, low bone-mass is associated with a lack of physical activity. The presence of arthritis, muscle pain, and weakness reduces mobility and impairs weight-bearing exercises. The WHO criteria makes it possible to assess the prevalence of osteoporosis in patients with lupus and to examine associated risk factors. Several cross-sectional and prospective studies in patients with systemic lupus have suggested a reduction of bone mass and an increased risk for fragility fractures. Therefore, it is advisable to measure bone mineral density in patients with lupus at risk for bone loss. Factors that play a role in the association between lupus and bone loss include the inflammatory disease itself, disease-related comorbidities and therapy, specifically exposure to glucocorticoids. Recognition of the role of cytokines and other inflammatory mediators in the regulation of bone remodeling has led to a better understanding of the etiology of bone loss in inflammatory diseases, including systemic lupus.
