Diagnostic value of hypotonic MRCP after oral water in low level biliary obstructive disease

Objective To explore the diagnostic value of hypotonic magnetic resonance cholangiopancreatography(MRCP) after oral water in the low level biliary obstructive disease.Methods MRCP images of 180 patients with low level biliary obstructive disease proven by surgical pathology or endoscopic biopsy pathology were analyzed retrospectively. All cases were studied with the conventional MRI, MRCP and the hypotonic MRCP after oral water. The manifestation and the diagnostic accuracy of two kind of MRCP for detecting the low level biliary obstructive disease were compared.Results The ability of hypotonic MRCP after oral water in depicting low level biliary obstructive disease was superior to that of conventional MRCP(P0.05). The qualitative diagnostic accuracy of two kind of MRCP for low level biliary obstructive disease was 72.8%(131/180) and 92.8%(167/180) respectively. The diagnostic accuracy of two kind of MRCP combined with other MRI sequences was 91.1%(164/180) and 96.1%(173/180) respectively. The consistency of two radiologist for diagnosing low level biliary obstructive disease was obviously improved using two kind of MRCP technique combined with other MRI sequences(κ=0.687,0.812). Conclusions There is a relative predominance in diagnosing low level biliary obstructive disease with hypotonic MRCP after oral water compared with conventional MRCP, it is an effectively supplementary method in detecting low level biliary obstructive disease to conventional MRI and MRCP.
