Laringectomía Supracricoidea como Procedimiento de Rescate Descripción de Caso y Revisión de la Literatura

2013
SUMMARY OBJECTIVE: The larynx plays a fundamental role in human communication; hence the management of cancer in this organ has the goal of curing patients, but also to preserve its function. It is estimated that local recurrence after organ preservation (based of chemotherapy and radiation therapy) protocols ranges from 16 % to 36 % and it is required the surgical salvage. The total laryngectomy continue today is the most commonly used procedure after the local relapse; however salvage partial laryngectomy have shown excellent survival outcome and local control in selected patients. CASE REPORT: Male patients 63 years old, diagnosed with squamous cell carcinoma of the larynx localized (glottic) and classified how stage II T2N0M0. Two years after the treatment the patient has local relapse, and he was treated with surgery procedure denominated laryngectomy supracricoid with cricohyoidoepiglottopexy, the clinical evolution of the patient was satisfactory, of the point of view functional and oncological. DISCUSION: The partial laryngectomy is the surgical procedure which we extirpate the tumor with adequate margins and this surgery preserves the speech, the oral feeding and avoids the permanent the use of the tracheostoma. The success of this approach depends always on the extent of the tumor, the surgical team experience and the proper selection of the cases. CONCLUSIONS: For his part the supracricoid laryngectomy is a procedure with good functional results and oncologically safe for the treatment of the local relapse after other therapies in selected patients.
