New oral anticoagulant drugs may improve the adherence of patients and of physicians to guidelines on the prophylaxis of venous thromboembolism in major orthopaedic surgery. However, before translating the results of clinical trials into daily clinical practice, more studies are needed, mainly in elderly patients and in those with a high risk of bleeding.
