Highlights •Atrial functional MR typically occurs in the context of AF and/or HFpEF. •Isolated annular dilation, insufficient leaflet growth, and impaired annular dynamics are mechanical culprits. •Early discrimination between atrial functional MR and secondary MR is pivotal to accommodate for different therapeutic needs. •Further study is needed to clarify the impact of early rhythm restoration strategies and mitral annular interventions to treat atrial functional MR.
