Ocular Myasthenia Gravis Onset: Diagnosis, Treatment and Course of the Disease (P6.314)

Background: Ocular myasthenia gravis (OMG) is defined by ophtalmoparesis and/or ptosis associated with laboratory findings (acetylcholine receptor antibody (AChR) or anti-MUSK) or electrophysiological findings (single fiber electromyography/ repetitive stimulation) and represents 15[percnt] of the total number of MG patients. Approximately in 50[percnt] of patients initially diagnosed with OMG, the disease became generalized in two years. Our aim is to describe the clinical features of patients diagnosed with OMG in our Center and the conversion rate to generalized MG. Methods: Retrospective descriptive analysis of a series of patients diagnosed with OMG. Demographic, clinical and therapeutic characteristics are described; and the conversion rate to generalized myasthenia gravis (II, III and IV) is analyzed. Results: 42 patients (54.8[percnt] women) with an average age of 55 (18.3DE) at the onset of OMG. Clinical presentation at the beginning was: 33[percnt] ophtalmoparesis, 31[percnt] ptosis and 35.7[percnt] both. 80.5 [percnt] presented ac´s anti-AChR-positive, 11.9[percnt] anti-MUSK positive and 4.8[percnt] both. Thymic pathology appeared in 21.4[percnt] (14.3[percnt] thymoma). The median follow-up was 5 years (1-27). 30[percnt] only needed cholinesterase inhibitors, 22.5[percnt] prednisone and 42.5[percnt] needed combination therapy (28.6[percnt] non-corticosteroid immunosuppressive agents), with symptomatic remission in 40[percnt] and 30[percnt] with minimal symptoms. During the course of the disease, 52.4[percnt] presented generalized forms of MG criteria. Conclusion: In our series, the most frequent clinical presentation was mixed (ophtalmoparesis and ptosis), most of the patients were anti-AChR positive, 70[percnt] clinically improved with treatment; and after five years of follow-up, in 52.4[percnt] of the cases, the disease became generalized.
