To study evidence for the involvement of a circulating ouabain-like factor in the genesis of essential hypertension, we measured, here cyto-chemical Na(+)-K(+) ATPase activity of erythrocyte in 31 hypertensive patients. The results were summarized as follows: 1) The mean arterial blood pressure(MAP) in patients with essential hypertension was 131.4±13. 9 mmHg, and that of control was 85.3±8.6 mmHg, with significant value in essential hypertension(p=0). 2) The Na(+)-K(+) ATPase activity of human erythrocyte membrane in the normotensive control was 0.2165±0.1800μ moles Pi/mg. Protein/hr. and that of hypertensive group was 0.1330±0.375μ mole Pi/mg. Protein/hr., with significantly lower values in hypertensive group(p=0). 3) When an erythrocyte membrane taken from normal human was incubated with supernatantes of boiled plasma from hypertensive individuals, the total and the Mg(++) ATPase activities of erythrocyte membrane were not different from those of normotensive control. However, the Na(+)-K(+) ATPase activity of erythrocyte membrane in the normotensive control was 0.1634±0.0427μ moles. Pi/mg. Protein/hr. and that of hypertensive group was 0.1200±0.0340μ mole Pi/mg. protein/hr, with significantly lower values in hypertensive group (p<0.001). 4) The percent inhibition of Na(+)-K(+) ATPase in the hypertensive group was significantly higher than that of normotensive control; the median values of % inhibition in the hypertensive group was 45.2%(range; 21.6%~76.5%) and that of normotensive was 21.7%(range; -17.5%~53.0%)(p<0.01). 5) The plasma digoxin measured by radioimmunoassay in hypertensive groups was 0.69±0.33ng/ml, and that of the normotensive control was 0.58±0.08ng/ml, without significant value. Based on the above findings, it is suggested that the pathogenesis of essential hypertension is primarily mediated by a circulating ouabain-like factor (or inhibitor of Na(+)-K(-) ATPase, inhibitor of Na(+) pump).
