Study on risk factors of plasma hyperosmolality in severe stroke

Objective To explore the risk factors of plasma hyperosmolality in severe stroke, and to provide evidence for therapy.Methods All the 80 patients suffering from severe stroke were divided into two groups according to the level of plasma osmolality: hyperosmolality group(310 mmol/L, 34 cases) and non-hyperosmolality group(≤310 mmol/L ,46 cases). All possible risk factors of plasma hyperosmolality were monitored. Results Seven valuable factors in single-factor analysis were analyzed by multi-factor Logistic regression analysis. High-dose mannitol(100 g/d), hypernatremia and hyperglycemia were the three main risk factors resulting in plasma hyperosmolality. The 4-week survival rate of hyperosmolality group(47.1%) was significantly lower than that of non-hyperosmolality group(78.3%, P0.01). Conclusions The main risk factors resulting in plasma hyperosmolality are high-dose mannitol, hypernatremia and hyperglycemia. Patients with plasma hyperosmolality have poor prognoses and low survival rates.
