Prophylactics of incompetence of the bronchus stump and bronchial fistulas after pneumonectomy

: The article is based on the materials of experimental and clinical investigations of healing the bronchus stump after pneumonectomy depending on different methods of surgical treatment, closure and cover. The course of postoperative period after pneumonectomy is analyzed in 249 patients. A system of prophylactic measures is proposed which considers the main links of pathogenesis of the complication. Using this system (including the modified Overholt method of treatment of the stump, closure with clips and covering with a pericardial pedicle flap as well as rational management of the cavity after pneumonectomy) allowed the frequency of bronchial fistulas to be considerably reduced.
