Study on the related factors of cognitive impairment in cerebral infarction patients with first attack

2011
Objective To study the related factors of cognitive impairment in cerebral infarction patients with first attack. Methods The lesion volumes and locations(frontal lobe 53 cases, parietal lobe 45 cases, temporal lobe 50 cases, occipital lobe 43 cases, thalamus 33 cases, basal ganglia 174 cases, cerebellum 30 cases and brainstem 8 cases) in patients(n=282) were measured by using CT/MRI techniques(all the patients were right-handed);Cognitive function were tested with Montreal Cognitive Assessment(MOCA) and Hamilton Depression Scale(HAMD).Results ① Cerebral infarction locus in frontal lobe, parietal lobe, temporal lobe, occipital lobe, thalamus and basal ganglia had correlation with obvious cognitive function impairment(P0.05).While,cerebral infarction of cerebellum and brainstem had no correlation with cognitive function impairment.② The lesion volumes had correlation with obvious cognitive function impairment(P0.05).③ There was a significant negative correlation between MOCA score and HAMD score(P0.05),and there were significant differences between normal cognitive function group and cognitive dysfunction group(P0.05).④ Cognitive impairment after stroke showed the features of low lever of education, while patients with long-term and great of alcohol consumption had significant differences of cognitive impairment (P0.05).Conclusion Cognitive dysfunction is a common complication of first-attack patients with cerebral infarction, and cognitive function impairment is related to the locus volumes and locations of cerebral stroke.Patients of first-attack with larger lesion volumes have more serious cognitive impairment, while patients with low lever of education,long-term and great of alcohol consumption or tristimania were more likely hadcognitive impairment.
