Clinical pathological study and prognosis analysis of preoperative radiotherapy and radiosensitivity in cervical cancer

Objective To evaluate the effect and clinical pathological influence of cirolemycin as radiosensitivity and radiotherapy on bulky cervical cancer at stage Ⅱa~Ⅱb1.Methods From January 1996 to October 2000,161 patients with bulky cervical cancer at stage Ⅱa~Ⅱb1 were divided into radiosensitivity+radiotherapy+operation group,radiotherapy+operation group and radical operation group.There was no significant difference among three groups in pathological type and clinical analysis,and they all received the same radiotherapy. The first group was given cirolemycin before intracavitary radiotherapy. The short term clinical therapeutic effect,histologic change,recurrent ratio,metastasis ratio and survival condition were compared among three groups.Results The tumor volume of the first group shrinked significantly when the radioation dosage was only half of regular cavity radiotherapy.The damage rate of cancer cells in deep muscular was 70.7%, significanly different from the other two groups(P0.05).According to histological observation,cancer tissue withered obviously,degenerate cell increased and histological grade upgraded in the first group before and after radiotherapy.No parametrial or vaginal stump was found. Lymph node metastasis rate had no noticeable statistical influence on the prognosis(P0 05).There was no significant difference in local recurrent rate and five year survival rate among these three groups(P0 01).Conclusions The preoperative radiosensitivity plus radiotherapy is effective for bulky cervical carcinoma.It is an effective method to kill malignant neoplasm cells by which the grade of tumor can be changed and the chances possibility of operation can be got.
