Abstract Objective Aim of the paper is to present the dynamics of changes in cognitive impairment in normal pressure hydrocephalus (NPH) in response to neurosurgical interventions. The role of neuropsychological examination was emphasized. Case Description applies to a female patient with a diagnosis of full-Hakim triad syndrome after two neurosurgical procedures. At each stage of the treatment cognitive assessment was performed. The results and interpretation were presented. Commentary Dynamics of cognitive dysfunction in normal pressure hydrocephalus show a high dependence on performed neurosurgical procedures, which regulate the pressure level within the ventricular system. Case study shows that the conduct of neurosurgery can stop progression of the disease, and thus – may raise the quality of life of the patient, although the effects of the treatment may also be unstable. From the perspective of correct classification and postoperative evaluation, neuropsychological assessment at the level of cognitive – behavioral functioning should be recognized as the standard in the diagnosis and monitoring patients with suspected normal pressure hydrocephalus.
