Infection with Neisseria gonorrhoeae has an incidence of 700.000 people annually, however gonococcal dissemination occurs only between 1% and 3% of cases. Gonococcal endocarditis occurs between 1% and 2% of disseminated gonococcal infections, occurring mainly in young men and is at high risk for complications and mortality. It is reported in the case of a 64-year-old man with fever and joint pain, a positive blood culture was obtained for Neisseria gonorrhoeae and in the echocardiogram vegetation was found in the mitral valve. Antibiotic management was given, which resolved bacteremia and vegetation.
