The article presents the results of the surgical treatment of 115 patients with complicated forms of cholelithiasis. The patients had the acquired disorders of bilioduodenal area and large concrements in the common bile duct. Depending on the surgical method the patients were divided into two groups. The control group included 54 patients with traditional surgical interventions. The study group comprised 61 patients, who were operated on biliary obstruction and choledocholithiasis using the percutaneous transhepatic endobiliary technique. The presence of endoscopic "difficult" choledocholithiasis and the inability to eliminate biliary obstruction with the retrograde endoscopic method were common to both groups. The percutaneous transhepatic endobiliary surgery of choledocholithiasis can be applied in patients with endoscopic "difficult" choledocholithiasis and high operational and anesthetic risk.
