Abstract Authors report a case of cholesterol granuloma of the petrous apex which was surgically treated by following an endoscopic approach. Based on this case authors review of the literature about recent management of cholesterol granulomas of the petrous apex. The tumor was approached endoscopically by a unilateral sphenoidotomy. Cholesterol granuloma was evacuated after localization of the carotid artery and the optic nerve. The patient recovered uneventfully. Transsphenoidal access to the petrous apex is a good alternative to lateral approaches. This acces is of choice when the cholesterol granuloma reaches the postero-lateral wall of the sphenoid sinus. The transsphenoid endoscopic approach, unlike lateral approaches to the petrous apex, does not disturb cochlear and vestibular function and allows postoperative endoscopic follow-up.
