Follow-up study of coronary arterial lesion due to Kawasaki disease

The time of appearance of obstructive lesions was studied based on 324 children with coronary arterial lesion due to Kawasaki disease, who have been followed-up by serial coronary arteriography (CAG) (3.2 9 1.6 times). The interval from the onset of the disease to CAG was divided into 7 periods; 2 yrs (242 patients), 24 yrs (87), 46 yrs (125), 68 yrs (81), 8I0 yrs (69), I012 yrs (32), 1214 yrs (26). Occlusion appeared mostly (44/47) within 4 yrs after the disease. 86% (54/63) of segmental stenosis appeared on right coronary artery (RCA). As to localized stenosis more than>75% (75%LS), the incidence of new appearance of it in each period on RCA decreased afterwards ( 7.9%, 2.3%, 0%, 1.2%, 0%, 3.1%, 0%, respectively). However that on left coronary artery (LCA) did not decrease ( 5.8%, 5.7%, 5.6%,! 6.2%, 5.8%, 12.5%, 7.7% ), and tend to progres~ to .~90%LS (55%) on subsequent CAG. Therefore, ~'90%LS appeared more frequently on LCA (7.7%) than that on RCA (1.9%). These results suggest that aneurysm on LCA should be followed carefully by CAG for a long period of time, even after aneurysm dicreased in size or disappeared on CAG. 36
