The present invention relates to a computer monitoring system (100) of a patient (P) capable of being afflicted with a particular disorder, including: - a computer processor (11, 21) configured to compare at least one clinical data (DC1, DC2) collected at a time t by a communication terminal (10) with at least one other clinical data (DC1, DC2) previously collected at a time t-1, to evaluate a degree of risk (SC) that the patient (P) is reached in said pathology in accordance with said comparison between said collected clinical data (DC1, DC2) and for generating a warning signal (AV) when the degree of risk (SC) is greater than a predetermined threshold - a central server (20) configured to transmit to said second communication terminal (30) a request to solicit an appointment with said practitioner, said request (RQ) containing at least one information relati ve the degree of risk (SC).
