Effects of Guided Care management model in risk prevention and control of drug-related adverse reactions among elderly patients

Objective
To explore the effects of Guided Care management model in risk prevention and control of drug-related adverse reactions among elderly patients.
Methods
Patients of Elderly Ward of Peking Union Medical College Hospital were selected as subjects by fixed point continuous sampling. Inpatients between January 2015 to December 2015 were chosen in control group. Inpatients between January 2016 to December 2016 were chosen in observation group. The Guided Care management model was carried out for risk prevention and control of drug-related adverse reactions among elderly patients from 2016 including risk assessment for drug-related adverse reactions with the Beers standard, finding problems, improving plan by multidisciplinary collaboration. The incidence of drug-related adverse reactions of inpatients was compared before and after implementation.
Results
The incidence of drug-related adverse reactions was 15.6% in 2015 and 7.6% in 2016 with a significant difference (χ2=86.78, P<0.01) .
Conclusions
The implementation of Guided Care management model can reduce the incidence of drug-related adverse reactions of elderly patients so as to guarantee nursing safety.
