Linfadenectomía pélvica extendida laparoscópica para cáncer de próstata

2008
Objective: The extent of lymph node dissection during radical prostatectomy has been a source of recent controversy. We evaluated the results of laparoscopic extended pelvic lymph node dissection in patients with high-risk prostate cancer. Methods: In a total of 9 selected patients with clinically localized prostate cancer, laparoscopic extended pelvic lymph node dissection was performed during radical prostatectomy. The approach was transperitoneal. The node dissection included the external iliac, obturator and hypogastric lymph nodes. Results: Extended dissection was unilateral in 6 patiens and bilateral in 3 patients. Mean operative time for the unilateral lymph node dissection was 45 min. No conversion was necessary. Related complications were not detected. Mean number of lymph nodes retrieved was 17 with extended bilateral dissection and 9 with extended unilateral
