Providing Care for a Changing CF Population

As the health of patients with cystic fibrosis has improved over the last 20 years, the need for a structured approach to transition from pediatric to adult care has become increasingly necessary. In many ways, CF experts have been at the forefront of advocating for a transition process. In this chapter, we review the unique aspects of CF as a multisystem disease, the impact of the Cystic Fibrosis Foundation, changing disease state in the setting of highly active CFTR modulator therapy, and recommendations for best practices in CF transition.
