Localização da margem dos preparos em restaurações estéticas: uma discussão em relação à saúde periodontal / Location of the final preparation in aesthetic restorations: a discussion related to periodontal health

Devido as necessidades esteticas muitas restauracoes apresentam margens com extensoes subgengivais. Porem, para a execucao de um trabalho estetico, uma serie de fatores deve ser considerada nao so com a finalidade de se obter a beleza estetica, mas tambem a saude dos tecidos de suporte envolvidos. O objetivo deste trabalho foi discutir a relacao entre a localizacao da margem dos preparos em restauracoes esteticas e a resposta dos tecidos periodontais. A metodologia utilizada foi a avaliacao de artigos pertinentes selecionados a partir da base de dados PubMed, alem da pesquisa manual. Apos a analise dos artigos, concluiu-se que: a) a margem supragengival e a melhor localizacao em termos de saude periodontal; b) restauracoes com margens subgengivais provocam danos aos tecidos periodontais; c) pacientes com higiene bucal adequada nao mostram reducao nos danos periodontais causados por restauracoes com margens subgengivais; d) a localizacao da margem dos preparos em restauracoes esteticas pode ser estendida subgengivalmente, desde que as medidas do tecido gengival supracrestal sejam verificadas e respeitadas contra lateral e individualmente. Termos de indexacao : Estetica dentaria. Periodontia. Saude bucal. ABSTRACT Due to the needs of aesthetic, many restorations have margins with subgingival extensions. However, in order to perform an aesthetic work, a number of factors should be considered not only for the purpose of obtaining the aesthetic beauty but also the health of the supportive tissues. The objective of this work was to discuss the relationship between the location of the preparation margins and periodontal tissue response in aesthetic restorations. The methodology used was the evaluation of relevant articles selected from the PubMed database, in addition to the manual search. After examining the articles it could be concluded that: 1 - the supragengival margin is the best location in terms of periodontal health, 2 - restorations with subgengival margins caused damage to the periodontium, 3 - patients with proper oral hygiene did not show reduction in periodontal damage caused by restorations with subgengival margins; 4 - The location of the preparation margins in aesthetic restorations may be extended subgingivally, provided that the supracrestal gingival tissue measurements are verified and respected contralaterally and individually.
