Hepatectomy combined with intraoperative choledochofiberscope for the treatment of intrahepatic bile ducts stone with liver cirrhosis

Objective To evaluate the applied value of hepatectomy combined with intraoperative choledochofiberscope for the treatment of intrahepatic bile ducts stone with liver cirrhosis.Methods Clinical data of 36 patients with intrahepatic bile ducts stone with liver cirrhosis undergone hepatectomy combined with intraoperative choledochofiberscope in our hospital from Jan 2001 to Dec 2009 were analysed retrospectively.Results There was 1 perioperative deaths in this series because of liver failure resulted from severe infections.The important postoperative complications occurred in 9 cases,including transient biliary fistula in 4 patients,subdiaphragmatic infections in 3 patients and hemobilia in 2 patients.Remaining bile ducts stone were found in 3 patients.32 cases were followed-up for 7 months to 9 years(average time 4 years).Stone recurred in 4 cases and 4 cases died.Among them 2 cases died of live failure,1 case died of massive bleeding from upper alimentary tract,1 case died of cholangiocellular carcinoma.Conclusion Hepatectomy combined with intraoperative choledochofiberscope for the treatment of intrahepatic bile ducts stone with liver cirrhosis can decrease the morbidity and operative mortality by reducing of the volume of hepatectomy and decrease the operation injury.Intensifying perioperative management is important to improve the prognosis.
