FIBRO-OSSEOUS AND RESPIRATORY EPITHELIAL HAMARTOMA OF THE FRONTO-ETHMOID REGION

A 31-year-old lady presented to the otorhinolaryngology clinic complaining of right exo - phtalmus. Imaging studies revealed a right frontal sinus mass, suggestive of a mucocele, and fronto- ethmoidal ground glass opacification. In the operating room, the frontal sinuses were exposed through an osteoplastic flap, and connected to the nasal cavity through a Lothrop-like drainage pathway. The pathologic analysis revealed a frontal sinus mucocele and fibro-osseous and respiratory epithelial hamartoma of the fronto-ethmoid region. In this case report, this rare pathology is discussed, in addition to a thorough elaboration upon the surgi- cal options as dictated by the anatomic complexity of the region. R E S U M E : Une patiente âgee de 31 ans s'est presentee au departement d'otorhinolaryngologie pour exophtal- mie droite. Les etudes radiologiques ont montre une opa- cification a type de verre depoli dans la region fronto- ethmoidienne, associee a une lesion du sinus frontal droit, suggerant une mucocele. Durant l'operation, les sinus frontaux ont ete exposes par un osteoplastic flap, et une communication a ete faite avec la cavite nasale par un drainage de type Lothrop. L'examen histologique a montre une mucocele du sinus frontal et un hamartome epitheliale respiratoire avec des zones de metaplasie fibro-osseuse dans la region fronto-ethmoidale. Cet article discutera cette pathologie rare, avec une mise au point sur les techniques chirugicales qui ont ete utilisees vu la complexite anatomique de la region en q u e s t i o n .
