System Barriers Associated With Diabetes Management in Primary Care

Abstract A review of the literature identifies system barriers that prevent primary care providers from efficiently and effectively managing their patients with diabetes. For optimal diabetes outcomes, system barriers must be addressed, and multi-stakeholder collaboration is needed. With passage of the Patient Protection and Affordable Care Act, new incentives exist to examine the applicability of the chronic care and patient-centered medical home models. Nurse practitioners need to be included to provide leadership in the redesign of the primary care delivery system in the care of chronic diseases like diabetes.
