The invention provides a method and a device for cutting a volume of interest of a three-dimensional medical image. The method for cutting a volume of interest of the medical image comprises the following steps: obtaining a three-dimensional medical image; obtaining initial seed points of a two-dimensional medical image; calculating in a preset range of the initial seed points in a current image layer and an adjacent image layer according to a preset calculation condition to obtain a final seed point; and obtaining a cutting result of the volume of interest of the three-dimensional medical image based on the final seed point. According to the technical scheme, the method and the device are capable of intelligently finding an optimal seed point for cutting the volume of interest according to the initial seed points provided by a user; the volume of interest of the three-dimensional medical image is cut based on the optimal seed point; the cutting accuracy of the volume of interest of the three-dimensional medical image can be effectively improved; furthermore, the cutting result of the volume of interest is stable; different cutting results caused by different positions of the seed points selected by the user can be avoided.
