Pathology of Cervical Specimen after LEEP and the Results of HPV Testing

Objective To explore the coincidence rate of cervical pathological diagnosis between loop electrosurgical excision procedure(LEEP) specimen and biopsy,and the positive rate of high risk human papilloma virus(HPV) infection before and after LEEP.Methods 397 cases were analyzed in this study.Pathological findings of post-LEEP and multiple biopsies were comparatively analyzed.HPV testing was performed and the positive rate of high risk HPV infection was analyzed before and after LEEP.Results The coincidence rate of cervical pathological diagnosis between LEEP specimen and biopsy was 49.87%(198 /397),39.29%(156 /397)was downgrade and 10.83%(43 /397) was upgrade in LEEP.The positive rate of high risk HPV infection was 23.67%(58 /245) in patients after LEEP and 86.00%(315 /368) in patients before LEEP.The difference between these two groups was significant(log-rank test,χ2=302.94,P=0.00).Among the 58 patients with persistent high risk HPV infection after LEEP,41(70.69%)patients developed recurrent disease during follow-up.However,recurrence was found in 11 of 51(21.57%) patients without infection.Conclusions The coincidence rate of cervical pathological diagnosis was high between LEEP specimen and biopsy.The positive rate of high risk HPV decreases after LEEP.Persistent high risk HPV infection is an indicator of risk of recurrence of CIN treated by the LEEP.
