Post-mastectomy Breast Reconstruction with Fat Grafting Alone

The authors describe their technique for post-mastectomy breast reconstruction using fat grafting alone in patients with flaccid, loose skin, via multiple injections of fat tissue. The technique comprises three stages: puckering stitches to remodel the breast after each fat graft; mastopexy and cone formation, creating and lifting a cone using the tissue from the area; neoformation of the inframammary fold, attaching the cone to the pectoralis major and the fold at the height of the sixth rib and performing a mini-lifting of the abdominal tissue to close the wound without causing tension. Using fat grafting and these three maneuvers, a satisfactory breast reconstruction can be obtained using only fat from the patient herself.
