Anterior instability of the shoulder due to bony glenoid defects and/or HillSachs lesions have recently received increasing attention and represent one possible reason for a recurrent anterior instability with or without isolated soft-tissue repair. In this review article, the newly developed concept of the glenoid track in the assessment of anterior shoulder instability is explained and different therapy options in the setting of significant bony glenoid defects, their pros and cons are discussed and illustrated by case examples.
