Big R, Easy M: Fracas Improvements with Robust Stakeholder Involvement

Failure Reporting, Analysis, and Corrective Action System (FRACAS) implementation has always had the potential to be a stumbling block. While FRACAS makes sense for Reliability engineering, the case for action is not always as clear to our customers (other engineering leaders, program managers, and external customers). An effective FRACAS pays close attention to the voice of its customers. All facets of a program benefit from a FRACAS with focus aligned with that of our customers by avoiding common mistakes. This paper documents a process for FRACAS implementation that helps solve these problems by integrating customers into FRACAS through upfront Key Performance Indicator (KPI) development and by ensuring continuous implementation and alignment of KPIs over time.
