The authors perfomed Nd:YAG laser Q-Switched confluent trabeculotomy in 167 eyes of 99 patients with uncontroled open angle glaucoma des pite maximal tolerated medical therapy. 25-45 shots of 7.8-15 mJ. were applied to the midtrabecular meshwork over 40-60 o . After a mean follow-up period of 18.9 ± 5.7 months, a blood reflux from the site of the trabeculotomy into the anterior chamber was observed in 35.3% of the 156 explored eyes. Our observations suggest the existence of patent holes througth the trabecular meshwork into Schlemm's canal. In this group of cases the mean intraocular pressure decreases and the mean outflow facility increases was significantly greater, and were associated with a larger frequency of successful treatment.
