Open Roux-en-Y Gastric Bypass

Open Roux-en-Y gastric bypass is a bariatric procedure, and as such it is indicated for severe obesity (body mass index of 40 kg/m2 or above without comorbidities or body mass index of 335 kg/m2 for those with comorbid conditions) refractory to dietary changes and medical treatment. Most of such patients are initially treated by a laparoscopic approach. Open Roux-en-Y gastric bypass is still required in some patients in whom the laparoscopic approach is not feasible. This chapter lists the indications, essential steps, common technical variations, and complications of this procedure. A detailed operative dictation template is included.
