The application of stereotactic electroencephalography in temporal lobe epilepsy

Objective To explore the values of stereotactic electroencephalography (SEEG) in localization of seizure focus in temporal lobe epilepsy. Methods From October 2014 to January 2016, 20 epilepsy patients who underwent SEEG for identification of seizure onset zones at the Department of Neurosurgery, China-Japan Friendship Hospital were enrolled. A retrospective analysis was conducted of surgeries and postoperative outcomes. Results Twenty patients underwent SEEG surgeries and 135 electrodes were implanted. There was intracranial hemorrhage about 3 ml in one electrode trajectory without any serious symptoms. There is no bleeding or electrode breakage forother electrode implantation. No patient experienced infection or cerebrospinal fluid leakage. The average error is 2.0±0.6 mm. In 14 patients, the seizure onset zones were located at hippocampus, parahippocampus or amygdala. In 6 patients, the seizure onset zones were located at the temporal neocortex. No patient experienced seizures in the follow-up period. Conclusions SEEG could help locate the seizure onset zones of temporal lobe epilepsy precisely and explore the epileptic brain networks in temporal lobe epilepsy with relatively low incidence of complications.
