Antibioterapia e insuficiencia renal aguda ¿causa o consecuencia?

espanolEl diagnostico diferencial de la insuficiencia renal aguda o subaguda es complejo, por la multi-tud de patologias a descartar. La nefritis tubuloin-tersticial aguda (NTIA) se asocia analiticamente a un fracaso renal agudo junto con eosinofilia (y eo-sinofiluria en un porcentaje de pacientes), clinica-mente puede correlacionarse con sindrome febril y datos de hipersensibilidad sistemica (rash ma-culopapular y artralgias). Entre sus causas encon-tramos reacciones de hipersensibilidad mediadas por farmacos (70%), enfermedades autoinmunes (lupus eritematoso sistemico), infecciones (15%), neoplasias malignas y formas idiopaticas (10%). La biopsia renal continua siendo la unica herramien-ta diagnostica confirmatoria. Por este motivo, su verdadera incidencia se encuentra subestimada, ya que se trata de pacientes de edad avanzada en los que se prefiere optar por un tratamiento em-pirico. La retirada precoz del farmaco sospechoso continua siendo la piedra angular del tratamiento. Si a estas importantes limitaciones anadimos una lista de farmacos potencialmente responsables en constante crecimiento, nos encontramos ante una entidad realmente desafiante. EnglishThe differential diagnosis of acute or subacu-te renal failure is complex, due to the multitude of pathologies to be ruled out. Acute tubulointerstitial nephritis (NTIA) is clinically associated with acute renal failure along with systemic hypersensitivity data (maculopapular rash, arthralgia and eosino-philia). Among its causes we find drug-mediated hypersensitivity reactions (70%), autoimmune di-seases (systemic lupus erythematosus), infections (15%), malignant neoplasms and idiopathic forms (10%). Renal biopsy remains the only reliable diag-nostic tool. For this reason, its true incidence is un-derestimated, since these are elderly patients in which it is preferred to opt for empirical treatment. The early withdrawal of the suspected drug remains the cornerstone of the treatment. If we add to these important limitations a list of potentially responsi-ble drugs in constant growth, we find a really cha-llenging entity.
