Abstract: Mixed germ cell malignancy is relatively rare in the prepubertal age group. The biologic behavior of mixed germ cell tumours follows that of its most malignant component. On one hand this tumour is known to have poor prognosis due to components like yolk sac tumour, on the other hand we are faced with the important task of preserving the fertility of the patient in this age group. The following case report is our attempt to extricate us out of this therapeutic dilemma by using a combination of conservative surgery with chemotherapy (Bleomycin, Etoposide and Cisplatin regimen). Keywords: Mixed germ cell tumour; Ovarian malignany; Prepubertal; Pelvic mass; Karyotype; Tumour markers; Conservative surgery; BEP regimen
