Introduction: Trigonocephaly consists in premature closure of the metopic suture, being the second most frequent type of craniosynostosis. This condition results in restricted growth of the frontal bone (keel forehead) and requires surgical treatment. Case presentation: A child, 7y30d, presented with cranial assimetry. In clinical and imaging investigation was found hypodevelopment of the frontal bone as well as medium frontal bony protrusion without any other change on the neurological exam. Discussion: Trigonocephaly is a type of craniosynostosis, a malformation of premature closure cranial sutures. The majority are isolated non-syndromic cases and involve only one suture. Diagnosis is clinically confirmed mainly by computed tomography (CT). The treatment of choice is surgical to prevent the progression of craniofacial deformities. The surgical technique chosen was zigzag coronal incision, osteotomy without orbital advancement and remodeling wiht no aesthetic compromises.This technique requires less surgical time and consequently minor blood loss. Conclusion: This report is relevant since numerous variations of surgical techniques have been proposed. Further studies are needed to improve the management of these malformations and to define the best surgical strategy to be employed.
