Comparative Study of Block Bone Graft with Chip Bone Graft in Herbert's Procedure for Old Scaphoid Fractures

In this study, we compared 17 patients with old scaphoid fracture that were treated with a block bone graft or a chip bone graft in combination with Herbert screw fixation. Nine patients underwent the block bone graft in combination with Herbert screw fixation (H+b). There were 7 males and 2 females, with an average age of 25.3 years, and an average follow-up period of 11.3 months. 8 patients underwent the chip bone graft in combination with Herbert screw fixation (H+c). They were 8 males, with an average age of 22.3 years, and an average follow-up period of 7.6 months. The following measurements were made for radiographical assessment: the angle of the radiolunate angle, the scapho-lunate angle. The radio-lunate angle increased from -7.3° to +0.6° and the scapholunate angle decreased from 66.7° to 60.8° in the H+b group. The DISI deformity improved in the H+b group. Postoperative bone union was obtained in all patients except for one who had been treated in the H+b group. The clinical results were satisfactory with both operative methods.
