Case report A g i r l w a s t r a n s f e r r e d f r o m t h e I n f e c t i o u s D i s e a s e s H o s p p i t a l t o t h e U n i v e r s i t y C l i n i c a l H o s p i t a l w i t h d i a g n o s i s o f d i a t h e s i s , d i s s e m i n a t e d i n t r a v a s c u l a r c o a g u l a t i o n ( D I C ) w i t h s u s p e c t e d s e p s i s . I n t h e h i s t o r y , s h e w a s d i a g n o s e d w i t h r u b e l l a 1 0 d a y s e a r l i e r , a s s h e h a d a c q u i r e d t h e r u b e l l a i n p f e c t i o n , p o s s i b l y f r o m h e r s i s t e r , a n d o n t h e d a y b e f o r e h o s p i t a l i z a t i o n s h e h a d h i g h f e v e r ( 3 9 ° C ) a n d p e t e c h i a e w e r e o b s e r v e d . O n a d m i s s i o n , t h e c h i l d ’ s c o n d i t i o n w a s s e v e r e , s h e h a d m u l t i p l e p e t e c h i a e o n t h e s k i n o f t h e e n t i r e b o d y a n d m a c u l o p a p u l a r r a s h . S i g n s o f d i a t h e s i s w e r e a l s o p r e s e n t i n t h e o r a l m u c o s a , w i t h o u t s i g n s o f a c t i v e b l e e d i n g . S I R S w a s d i a g n o s e d b a s e d o n t a c h y c a r d i a ( 1 3 0 b p m ) , t a c h y p n o e a ( 4 5 – 5 0 / m i n ) , h i g h f e v e r ( 3 9 – 4 0 o C ) a n d l e u k o c y t o s i s [ 7 ] . M o r e o v e r , t h e p a t i e n t h a d g e n e r a l i z e d e d e m a , h e r b l o o d p r e s s u r e w a s d e c r e a s e d . L a b o r a t o r y t e s t s h o w e d , a l s o m i l d a n e m i a , t h r o m b o c y t o p e n i a , D I C a n d i n c r e a s e d a c u t e p h a s e m a r k e r s ; t h e r e n a l f u n c t i o n t e s t s w e r e n o r m a l ( T a b l e 1 ) . D u e t o t h e a b o v e , t h e i n i t i a l d i a g n o s i s o f s e p s i s w a s m a i n p t a i n e d . W i d e p s p e c t r u m a n t i b i o t i c t h e r a p y w a s c o n t i n u e d ( m e r o p e n e m , c e f t r i a x o n e ) ; s h e a l s o r e c e i v e d i m m u n o g l o b u p l i n s ( 0 . 2 5 g / k g ) , p l a s m a , p l a t e l e t c o n c e n t r a t e a n d a l b u m i n , a n d f u r o s e m i d e . T h e d i a g n o s i s o f s e p s i s w a s r u l e d o u t i n t h e f o l l o w i n g d a y s d u e t o n e g a t i v e r e s u l t o f c u l t u r e , r e l a p t i v e l y l o w v a l u e s o f a c u t e p h a s e m a r k e r s a n d l a c k o f e f f e c p t i v e n e s s o f w i d e p s p e c t r u m a n t i b i o t i c t h e r a p y . O n t h e 4 t h d a y o f t r e a t m e n t , f e v e r w a s r e s o l v e d . H o w e v p e r , t h e c o n d i t i o n o f t h e c h i l d d i d n o t i m p r o v e d , s i n e s h e h a d a w o r s e n i n g d y s p n o e a , w i t h a d r o p i n s a t u r a t i o n t o 6 0 % . T h e l u n g X p r a y s h o w e d i n f l a m m a t i o n a n d a c u t e r e s p i p r a t o r y d i s t r e s s s y n d r o m e ( A R D S ) . I n a d d i t i o n , s h e d e v e l p o p e d b l e e d i n g f r o m t h e u r i n a r y t r a c t . F o l l o w p u p l a b o r a t o r y t e s t s s h o w e d i n c r e a s i n g a n e m i a w i t h h i g h r e t i c u l o c y t o s i s , s l i g h t i n c r e a s e i n b i l i r u b i n c o n c e n t r a t i o n s ( m a i n l y u n c o n j u p g a t e d ) , a n d i n c r e a s e i n f r e e s e r u m h e m o g l o b i n c o n c e n t r a p t i o n s , a s w e l l a s h i g h L D H a c t i v i t y ( T a b l e 1 ) . T h e r e f o r e , s u s p e c t e d d i a g n o s e s o f t h r o m b o t i c t h r o m b o c y t o p e n i c p u r p p u r a ( T T P ) o r h e m o l y t i c p u r e m i c s y n d r o m e ( H U S ) w e r e m a d e ; h o w e v e r , t h e y w e r e e v e n t u a l l y r e j e c t e d d u e t o t h e l a c k o f s i g n s o f r e n a l f a i l u r e , n e u r o l o g i c a l s y m p t o m s , a n d t h e r e w e r e n o s c h i z o c y t e s i n b l o o d s m e a r . B o n e m a r r o w e x a m i p n a t i o n s h o w e d i n d i r e c t l y a c u t e i m m u n e t h r o m b o c y t o p e n i a . A d e c i s i o n w a s m a d e t o i n t r o d u c e c o m b i n e d t r e a t m e n t w i t h g l u c o c o r t i c o s t e r o i d s ( 2 m g / k g / d a y ) a n d i m m u n o g l o b u l i n s ( 0 . 7 5 g / k g ) .
