Future Health Service Model in the Context of Decentralization (การจัดบริการสาธารณสุขในอนาคตภายใต้บริบทของการกระจายอำนาจ)

The purpose of this study was to identify the future health service model under the Decentralization Act B.E. 2542. It is crucial that decentralization at the optimal level increases economy of scale. This paper had proposed 2 option.First,the transferring of local health service units to local government should be applied only to the potential local government which could completely handle the unfragmented health system.In case of the tertiary care units which requries hight technology and investmented, they should remain to be under the Ministry of Public Health. And for health service units in Bangkok,they might be either transferred to the Bangkok Metropolitan Administration or still be part of the Ministry of Public Health (MOPH). The local authority will be,in the latter;a purchaser and MOPH, a provider.The second option was to partially transfer some health service i.e. health promotion and disease control to only capable local authorities whenever feasible.
