Akut Böbrek Hasarının Erken Tanısında Plazma NGAL (Neutrophil Gelatinase-Associated Lipocalin) Etkinliğinin Off-pump ve On-pump Kalp Cerrahisinde Karşılaştırılması

Comparative Efficiency of Plasma NGAL (Neutrophil Gelatinase-Associated Lipocalin) in Off-pump and On-pump Cardiac Surgery in the Early Diagnosis of Acute Renal InjuryObjective: Development of acute kidney injury and acute renal failure after cardiac surgery results in increased rates of mortality and morbidity, and prolonged hospital stay. In our investigation our aim was to predict the occurrence of acute renal injury which is one the frequently seen complications after cardiac surgery.Material and Methods: The study included 40 patients who had undergone (on-pump) and CPB (off-pump) CABG surgery. Patients were divided into on-pump (Group A: n=20), and off-pump (Group B: n=20) CABG groups. Plasma neutrophil gelatinase-associated lipocalin (pNGAL) levels were determined at baseline and 3. (T), 12. (T) and 24. (T) hours after cardiac surgery. Results: Plasma levels of T and T NGAL were significantly higher in the on-pump cardiac surgery group when compared with the off-pump cardiac surgery group (T p:0.023 and T p:0.040). There is no statistically significant differencein plasma creatinine levels between two groups (p>0.05). Conclusion: Based on our study, we think that despite acute changes in serum creatinine and renal pNGAL levels in on-pump and off-pump patients two surgical methods do not differ in terms of development of renal damage, and mortality, and morbidity rates, and also further larger scale studies comparing both surgical modaliti,es are needed.
