The purpose of a prosthetic implant is to reproduce anatomy as normal as possible and properly functioning, to correct any defects that may have caused joint degeneration or failure of a previously placed implant. Ideal prostheses, in fact, should not only replace an articulation but must be integrated into the complicated load transfer system from the trunk to the limbs. The causes of prosthetic implant failure are several but in up to 77% of cases responsibility can be attributed to a biomechanical problem. To give longer life to our implant, it is therefore necessary to know and apply rules for the recovery of biomechanical parameters. At the disposal of the surgeon in the preoperative phase there are lots of investigation, tools and applications that prevent possible complications, predict the positioning of the implant in all its components so that it can also handle the surgical options available with surgery. Accurate planning with the use of current technologies will help to minimise potential implant-related problems. The essence of preoperative planning in hip surgery can simply be summed in two words: be prepared.
