Does the Grade of Diastolic Dysfunction Predict Short-term Outcomes Of Acute Ischemic Stroke Patients? (P3.075)

Objective: To determine if diastolic dysfunction (DD) in acute ischemic stroke (AIS) patients is associated with differences in short-term outcomes. Background: DD is a common finding in patients who suffer AIS. Data describing the consequences of DD in AIS patients are lacking. Design/Methods: Patients diagnosed with AIS presenting to our center between 6/2008-5/2014 and assessed by transthoracic echocardiogram (TTE) were identified from our prospective registry. Patients with in-hospital stroke or with systolic dysfunction (EF 2) and unfavorable discharge (other than home or inpatient rehabilitation) were investigated by degree of DD. Results: Of the 1,583 patients screened, 691 met criteria (median age 65 years; 67[percnt] black; 46[percnt] female). Of these, 147 (21.2[percnt]) had no DD, 478 (76.48[percnt]) had grade 1 DD, and 66 (12.13[percnt]) had grades 2-4 DD. After adjustment for covariates, no differences in outcomes were detected comparing no DD with DD grade 1 patients. Multivariate analysis adjusting for age, NIHSS at baseline, glucose at baseline, and tPA administration, found that patients with DD grades 2-4 were over four times more likely to experience in-hospital mortality (OR=4.32, 95[percnt] CI 1.57-11.90, p=0.005) than patients with grade 1 DD and were five times more likely to experience in-hospital mortality (OR=5.04, 95[percnt] CI 1.23-20.67, p=0.025) than patients with no DD. Conclusions: After adjustment for covariates, DD grade 1 was not associated with death or poor functional outcome.
