Ganz periacetabular osteotomy through the iliac-inguinai approach for treatment of acetabular dysplasia in elder children

Objective To introduce Ganz periacetabular osteotomy through the iliac-inguinal approach for treatment of acetabular dysplasia in elder children. Methods We retrospectively reviewed the records of 10 cases (13 hips) of acetabular dysplasia from Oct. of 2001 to Apr. of 2006. Surgical technique and application of Ganz periacetabular osteotomy through the lilac-inguinal approach were described in detail. Results All cases were followed-up for 31 months in average. Bony healing were completed 3 months after operation. Symptoms disappeared and motions of the hip joint were good postoperatively. Radiographic appearances were improved significantly. The inclination acetabular angle decreased 25 in average, the angulus acetabularis decreased 27 , the CE angle decreased 32 , Acetabular-Head-Index increased 41% in average. Conclusions Ganz periacetabular osteotomy is characterized by complete exploration, safe and exact osteotomy, and reliable corrective result. It refers to acetabular dysplasia in children with closed triradiate cartilage. However, this operation has massive surgical trauma, the safety of perioperative management should be emphasized.
