Effect of Paraoxonase-1 gene polymorphism on multiple organ dysfunction syndrome induced by acute organic phosphorus poisoning

Objective
To investigate the correlation between paraoxonase 1 (PON-1) gene polymorphisms and multiple organ dysfunction syndrome induced by acute organic phosphorus poisoning (AOPP).
Methods
A retrospective study was done on clinical data of 140 patients with AOPP admitted into Central Hospital of Zhanjiang, Guangdong Province, from January 2011 to October 2014. The subtypes of PON-1 gene polymorphisms were tested by PCR technique and the patients were grouped. The sequential organ failure score (SOFA), cute physiology and chronic health status score (APACHEⅡ) and the incidence of multiple organ dysfunction syndrome (MODS) were analyzed and compared in different groups.
Results
In patients with AOPP, the PON-1 genotype included Q/Q, Q/R and R/R subtypes. The score of SOFA was (7.1±1.6) points for Q/Q subtype, (6.8±1.1) points for Q/R subtype, (6.3±1.2) points for R/R subtype (F=3.30, P<0.05). APACHEⅡ score was (25.8±4.4) points, (24.6±3.8) points and (23.3±3.6) points for Q/Q subtype, Q/R subtype and R/R subtype (F=3.47, P<0.05). APACHE II in Q/Q genotype group were higher than that in Q/R and R/R genotype groups, and the difference was statistically significant. The incidences of MODS for Q/Q, Q/R and R/R subtypes were 30%, 13.92% and 6.45%, respectively (F=6.82, P<0.05).
Conclusion
PON-1 gene polymorphisms affect the activity of PON-1, which leads to the degradation of organophosphorous pesticide and hence result in multiple organ failure.
