The Ongoing Evolution of Liver Transplantation in Alcohol-Associated Liver Disease.

Liver transplantation (LT) for alcohol-associated liver disease (ALD) has now become the most common indication for LT.(1). Patients with alcohol use disorder (AUD) have excellent outcomes after LT with most recipients remaining abstinent or drinking in a moderate fashion(2). The "6-month" rule, which imposed a fixed interval of sobriety prior to LT consideration, has increasingly been abandoned as scientific evidence demonstrates such intervals to be, at best, a weak predictor of future harmful alcohol relapse and arbitrarily excluding many patients who would do well with LT and who will almost certainly die otherwise(3).
